If you have completed the 6 month assessment data and are waiting for the previous team to lock the record to discharge, you should email the previous team and ask them to lock the record to discharge and transfer the record to you via the 6 month assessment tool.
Contact information for all teams on SSNAP can be found here: www.strokeaudit.org > Resources > Team codes and contacts. If you aren’t sure which team was the last team to enter data, please email the SSNAP helpdesk (ssnap@kcl.ac.uk) with the SSNAP ID number of the record in question.

Dear ,
I am completing the 6 month assessment data for SSNAP ID: XXXXXXX. This record is currently with your team (Team Name) and has not yet been locked to discharge and transferred to our team (Team Name) via the 6 month assessment tool (https://ssnap.zendesk.com/hc/en-us/articles/360017063497-How-do-I-transfer-a-record-to-another-team-to-complete-the-6-month-assessment-). Please could you transfer this record to us at your earliest convenience.
