The search tool allows 6 month assessments to be entered regardless of whether the record has been discharged and locked on SSNAP. The 6 month assessment can then be saved, and transferred at a later date once the record has been discharged and locked by the previous SSNAP team.

After selecting the 'Enter and transfer records' page,you should select the team performing the 6 month assessment. You can then either search for patients ready for their 6 month assessment, or view a list of the 6 month assessments which you have already entered for this team.
When searching for a patient who is ready for their 6 month assessment, you will need to enter the Patient audit number and the NHS number. If you do not have both of these, there is an option to enter extra information.
Once you have entered the 6 month assessment data, if the record has been locked to discharge you can immediately transfer the record to your team. If the record has not been locked to discharge, the record will be stored, and you can access this and amend it at any time in the Find Patient > List Six month assessments already entered area. Once the record has been locked to discharge by a previous team, the ‘transfer’ button will become available, and you will be able to transfer the 6 month assessment to your team, and then subsequently lock the record.
Please ensure that you have locked your 6 month assessment after it has been transferred.
If you are transferring a record to a 6 month assessment team which you are not a part of, you can do this by ticking the Show all teams checkbox. This will allow you to select the team performing the 6 month assessment, enter the patient details, and then transfer the record.
