The 'Records ready for assessment' report can be used to find which patients are due their 6 month assessment, or which patients have not yet had their 6 month assessment recorded on SSNAP. This report can also be used to find records which have been fully completed, but not yet locked by your team.

Once on the unlocked records page, select your team and the relevant date range. Entering a date range is useful if you are seeking records from a specific period of time, for example, searching for records with a clock start of 4-8 months before the current date when searching for 6 month assessments which are due now.
After selecting ‘See patient IDs for unlocked records’ you will then be able to see records listed under a variety of headings. The most important for 6 month reviews are the 8 months from onset/arrival not fully locked to 6 months headings. This heading provides a list of patient IDs for records which are 8 months (or longer) from clock start, but have not been completed and locked up to 6 months. There is a heading for both transferred and not transferred records.
You can then click on these IDs to view the patient records – you will then be able to complete 6 month assessment information required for each of these records before locking to 6 months.
