Ambulance teams should only enter data for patients that they conveyed to the hospital.
Ambulance teams that who find that a record has been assigned incorrectly, should make a note of the number of records and add this information when submitting their return (see example below). Ambulance teams can contact the hospital team and request that the record is amended accordingly.
