What these tabs show
The L11, L12 and L13 tabs present information about the wider rehabilitation and support provided by community stroke services beyond core clinical therapy. They show:
- support for healthy living and lifestyle management,
- social care and care delivery activity, and
- additional or “other” forms of rehabilitation input.
Together, these measures help services understand the breadth of community support offered to patients during their time with the service.
L11 – Healthy Living and Lifestyle Management
What this tab shows
The L11 tab provides information on rehabilitation activity aimed at supporting patients with healthy living and lifestyle changes while under the care of the community service.
Key metrics
The percentage of patients identified as requiring healthy living and lifestyle management during their time with the service
The team figure shows the proportion of patients with this identified need.
The national figure provides a comparison across all community teams.
Variation may reflect differences in patient needs or local service focus.
The median number of minutes of healthy living and lifestyle management received over a patient’s total stay with the service
The team value reflects the typical amount of input delivered locally.
The national value provides a benchmark across all community teams.
Differences may reflect variation in service delivery models or caseload mix.
