What this tab shows:
This tab provides a comprehensive picture of the changes measured between the first assessment and discharge/end of active therapy of all patients discharged from the community team during this period. Information about patients who were considered to require active therapy during their stay with the service is provided in tab L6. Active therapy cohort. Tab L4 Outcome Measures focuses on three key metrics:
1. Modified Rankin Scale (mRS) — L27.1 to L28.40
The mRS is a 0–6 scale measuring disability and independence. Changes between the first assessment and discharge indicate whether a patient’s condition has:
- Improved
- Stayed the same
- Worsened
This gives an overall view of functional progress during community rehabilitation.
2. Barthel Index — L29.1 to L30.17
The Barthel Index measures ability to perform Activities of Daily Living (ADLs).
It provides a more detailed and task‑specific view of functional change than mRS. It reflects how well a patient can manage practical daily tasks, and improvements indicate a growing level of independence in everyday life.
3. EQ‑5D‑5L — L31.1 to L32.68
The EQ‑5D‑5L is a standardised health‑related quality‑of‑life (HRQoL) measure used to capture a patient’s own view of their health. It has two main parts:
- Five dimensions (EQ‑5D)
- Five severity levels (the “5L” part)
What EQ‑5D‑5L shows in SSNAP
- How patients perceive their health at the start and end of community rehabilitation.
- Changes in mobility, independence, mood, pain, and daily functioning.
- A patient‑centred view of recovery that complements clinical measures (e.g., mRS, NIHSS, Barthel).
Because the measure is standardised, it can be used to compare outcomes across teams, regions and services.
