The L6 Active Therapy Cohort tab focuses on patients who were considered to require therapy during their time with the community stroke service. It provides information on:
- the group of patients identified as needing therapy,
- the amount of rehabilitation and therapy they received, and
- how that therapy was delivered.
All measures in this tab relate only to patients recorded as requiring at least one type of therapy during their stay.

Therapy and Rehabilitation
Key metrics in this tab
Percentage of patients who were considered to require active therapy during their stay with this service – L37.1–L37.3
- Identifies the cohort of patients who were considered to require active therapy while under the care of the service.
- Includes patients needing one or more areas of therapy such as motor, psychological, communication/swallowing, or vocational rehabilitation.
- All other measures in this tab are reported for this group only.
Median minutes of rehabilitation (all domains) received for total stay with this service – L38.1–L38.3
- Shows the median of total amount of rehabilitation received by patients during their entire stay with the service.
- Covers all forms of rehabilitation support provided within community care.
Median minutes of total 'active therapy' (therapy domains only) – L39.1–L39.3
- Shows the median total minutes of active therapy received during the patient’s time with the service.
- Reflects therapy across the main clinical active therapy types.
Methods of active therapy delivery – L39.4–L39.12
- Shows the proportion of therapy that was delivered by rehabilitation assistants, via telerehabilitation or within a group session
Therapy during block 1-6 – L40.1–L45.7
- Shows the median minutes of active therapy received by patients during each block at this service.
- Shows the length of time patients receiving active therapy remained with the service during block 1-6.
- Reflects the change in intensity of therapy across the community episode.
Therapy outcomes
These KIs show changes in patient outcomes between first assessment and discharge / end of active therapy, shown for both the team and the national position.
Modified Rankin Scale (mRS) at first assessment and discharge
- Shows the distribution of patient disability levels when first assessed by the service and at discharge/end of active therapy.
- Allows comparison between the team and national profiles at both time points.
Change in mRS score from first assessment to discharge
- Shows how patients’ mRS scores changed over the course of therapy.
- Improvements, no change, and deterioration are displayed as proportions of patients.
- Enables comparison of outcome patterns between the team and national figures.
- Provides a high-level view of functional change associated with community therapy input.
Barthel Index at first assessment and discharge
- Shows patient independence levels at first assessment and at discharge/end of active therapy.
- Highlights changes in functional ability over time, from greater dependence to increased independence.
- Allows comparison of the team’s patient profile with national distributions.
- Complements mRS by focusing specifically on activities of daily living.

- understand the size and characteristics of the active therapy population,
- review the overall amount of rehabilitation and therapy provided,
- explore different methods of therapy delivery, and
- consider how therapy is provided over time within the community pathway.
