Click on the clinical tab, followed by 'Patient Records'.
Ensuring that the correct team is selected in the 'Team' dropdown box, you can then click 'Add case'.
Once clicking on this, you will be presented with the following page:
Once you have filled in the above initial fields, press the 'Save' button, and a record will then be created with a unique SSNAP Patient ID.
Tip: If you need to contact the helpdesk regarding a record, please only use the SSNAP Patient ID, do not include any patient identifiable information. SSNAP cannot view the name, NHS number or date of birth.
