What should I do if a record was an inter-facility transfer?
If a record assigned to your ambulance trust was actually an inter-facility transfer then this can be recorded as such in Q1.12.0c. When 'Yes' is selected to Q1.12.0c, the remaining questions do not need to be completed and the record can be locked.
2. What should I do for records incorrectly assigned to my ambulance service?
Ambulance teams should only enter data for patients that they conveyed to the hospital.
Ambulance teams that find that a record has been assigned incorrectly, should make a note of the record then contact the hospital team and request that the record is amended accordingly.
Team codes and contacts are available to all registered users, this can be found on the web tool by going to Resources > Team Codes and Contacts: SSNAP - Team codes and contacts (strokeaudit.org)
3. How do I complete records for patients who were not identified as stroke by my ambulance service?
For patients not initially recognised as a stroke by your ambulance service: Teams should still enter data for these cases as normal. The clinical impression will be ‘other’ and you can email the SSNAP helpdesk - ssnap@kcl.ac.uk to report this.
4. What happens with incomplete ambulance records?
By each submission deadline, if you are unable to enter the complete data for an ambulance record, this record will not be included in your ambulance team’s reporting.

5. Which patients can Ambulance Trusts enter data for?
Ambulance trusts will only link and enter data for those patients with a SSNAP record already started.
For an acute teams, please ensure that you accurately enter information on Question 1.12.1 so that the correct ambulance trust can be identified. The CAD number will be validated as part of this process and once permission has been granted for ambulance teams to see identifiers they will be able to see NHS number and other patient details.
6. What is my ambulance team code?
The ambulance team code is a unique three-character code specific to your ambulance service. All ambulance team codes are listed below:
