Overview
The new 6M Look Up Tool is designed to streamline the process of finding and completing 6-month assessments independently of hospital and community team record status. This guide walks you through how to use the tool, key differences from the previous method, and how to finalise and lock records.
Accessing the 6M Look Up Tool
To access the tool:

Navigate to new.strokeaudit.org

Go to Your team > Six Month Dataset > Study Records > Add record
- Then press 'Look Up'
Searching for a Patient Record
Matching Criteria
To locate a patient’s record, enter at least one of the following then press search:

SSNAP ID (Patient Audit Number)

NHS Number
If one of them is not available, you must provide extra identifying details:
Extra information is required. Please answer one of the following (3a, 3b, or 3c):
3a. Date and time of arrival at first hospital:

AND
Date of onset:

3c. Two of the following:

Once you have successfully looked up a record, section 1 will be auto-completed with the patient details as below:
Completing and Locking a Record
Once you have found the patient’s record:

Enter the 6-month assessment data in section 8.

Save your progress.

Once complete, lock the record to finalize the entry.
4. Make sure you lock the record as above and then the status will be locked.
