Who should start a SSNAP record?
The team that first admits the patient should start the SSNAP record.
The only exception is for patients who receive IA intervention.
What records should be added onto SSNAP?
Patients with a primary diagnosis of stroke coded as I61, I63 or I64 should be added onto SSNAP. Additionally, any patient who follows the stroke care pathway should be included on SSNAP. If a patient has a serious co-morbidity that would prevent them from following the stroke care pathway, these patients should be excluded from the SSNAP dataset.
The minimum age for patient submission to SSNAP is 16.
Patient had the stroke up to 28 days before presenting to Hospital (more than 28 days, excluded).
Patients who are not admitted to hospital (for example if treated and discharged from A&E OR died in A&E) cannot be entered onto SSNAP. We would recommend capturing information on these patients at local level.
How to create a new record on the webtool
Click on 'Sites', then select the dataset from which team you are trying to register the patient with. Also, select the PEN ICON to access the dataset
Ensuring that the correct team and dataset is selected, you can then click 'Add record'.
(You do not select Stroke dataset/Other dataset /TIA dataset at this stage)
Once clicking on this, you will be presented with the following page:
Once you have filled in the above initial fields, press the 'Save & validate' button, and a record will then be created with a unique SSNAP Patient ID.
You will then be able to complete section 1-7.
Finding and editing existing records:
To find an edit an existing record you can use the search function:
You can search by patient audit number, hospital number, NHS number, forename or surname.
Please be aware, we have heard your feedback regarding the search function and expect to implement further improvements over time.
Tip: If you need to contact the helpdesk regarding a record, please only use the SSNAP Patient ID, do not include any patient identifiable information. SSNAP cannot view the name, NHS number or date of birth.
