Our aim is for SSNAP data to be used as widely as possible to improve the quality of care provided to people with stroke.
The Healthcare Quality Improvement Partnership (HQIP) is the data controller for SSNAP. HQIP is therefore responsible for deciding whether SSNAP data can be shared.
Types of data available
The same initial data request questionnaire can be used to request either of the following:
Aggregate data
Data presented at the level of an individual hospital or team.
Anonymous patient-level data
Data presented at the level of individual patients, with personal information removed. This does not include information such as names, dates of birth, addresses or NHS numbers.
How to submit a data request
1. Review the available documentation

2. Complete the initial questionnaire

The questionnaire asks for information about your proposed project, including its context, methodology and funding.
Once the Stroke Programme receives your completed questionnaire, the request will undergo an initial screening. We will consider whether the proposal is technically feasible and whether it can proceed to the formal application stage.
Please note: SSNAP will charge a review fee if a request progresses beyond the initial screening stage.
3. Complete the HQIP application documents
If your request passes the initial screening, you will need to complete:
- A Data Access Request Form (DARF)
- A Data Sharing Agreement

The completed documents must first be returned to SSNAP for consideration and signing. They can then be submitted to HQIP for formal review.
4. HQIP reviews the application
Applications are considered by HQIP’s Data Access Request Group (DARG), which normally meets once a month.

Please note: If an application is approved, additional fees will apply to the preparation and sharing of the requested data.
