SSNAP allows teams to add custom additional questions to their proforma. This is a great resource for local data requirements, and local audits.
It is important you have thought clearly about the questions you are inputting. Ensure that you consider the added functionality of adding answer options and validations prior to adding additional questions to the webtool
This section appears as a new tab titled "Additional Questions for all teams" or "Additional questions for X team" within patient records and contains questions relevant to various local audits.
Additional questions for all teams
Some questions are available to all teams, this is where current studies require multiple teams to complete these questions. Only the teams participating in the relevant study are expected to answer them.
If Your Team is NOT Part of These Studies
If your team does not participate in any of the local audits associated with these additional questions, you can simply ignore Additional Questions for All Teams tab. You can continue to lock and transfer patient records as you normally would without any required action.
If Your Team IS Part of These Studies
For teams involved in one of these studies, you only need to answer the specific questions relevant to your study. There is no requirement to complete questions outside your study's scope.
Additional questions for a specific team
In addition to this, you can request that additional questions are added to your team alone. These questions would then be available in another tab "Additional questions for team X"

The information required is
- The question/s to be added
- The question answer options
- The answer format (e.g. free text, dropdown list - for a full list of options see below)
- The team which will need access to the questions.
Answer formats available:
Text – This a free text option where users can determine what to type into the box, this is useful when the range of answer options is too wide to define.
Radio button list – This option allows users to select one option that best describes the answer to the
question.
Drop down list – If the values you want to be entered correspond with a set list, a dropdown menu may be appropriate. An example of the dropdown list is choosing the question type itself. Only set options are available. Such as Text/Radio button list etc.
Numeric – This option is for questions that should only have a numerical value. For example, weight, days, hours, millimetres etc. You can set the number of decimal places (maximum of 3) and also maximum and minimum values.
Date – This option is appropriate for asking about dates, times can also be entered by asked as separate questions. A date box will appear but they can also be entered manually in the following format dd/mm/yyyy
Time – Times can be used for specific timings. They also have a time box slider that appears but can be entered manually in 24 hour clock as such ‘23:00’.
Check Box – You would use this option if users are able to select multiple preset options. Each answer input on the additional questions section represents a column in the export. For this reason each check box answer option represents a column and all options must be entered separately. This will then appear as a “TRUE” or “FALSE” depending if the box is ticked or un-ticked.
Do not use this function if you only need one option selected, if you have a Yes/No question please either use a dropdown menu or radio buttons. It will be much easier to analyse!
Impact on SSNAP Exports
Exports of SSNAP data will now include extra columns corresponding to the questions in Additional Questions for All Teams tab. However, the export file specification has not changed. This means you can safely delete the additional question columns from the exported file without affecting your existing data processing workflows.
