An Early Supported Discharge team (ESD) refers to a service delivered by specialist professionals within a multi-disciplinary team to provide rehabilitation and support in a community setting with the aim of reducing the duration of hospital care for stroke patients and enabling them to return home quicker.
A specialist community rehabilitation team (CRT) refers to a service delivered by specialist professionals within a multi-disciplinary team working in the community delivering rehabilitation services within a patient’s home. A community rehabilitation team (CRT) will cater for patients following inpatient rehabilitation or transfer from Early Supported Discharge (ESD).
