In order to transfer the 6-month assessment there needs to be an existing record from the beginning of the patient's stroke pathway. Only acute teams can add the patients onto SSNAP, if the acute team does not add the patient onto SSNAP the six month assessment cannot be completed.
Transferring a record for general community data and 6-month assessment data
- If you are transferring to a community team for them to enter their therapy information, then you would transfer to them via section 7. This would mean that the community team will receive a record where they can enter data regarding cared receive in the community information in section 4 and 7. They will also be able to enter the 6-month assessments if this is relevant to them.
Transferring for 6-month assessment data only
- If you are transferring the record for a 6-month assessment ONLY this should NOT be done via section 7. If you transfer the record via section 7 the 6-month provider will have to complete sections 4 and 7 which is not relevant to them. To transfer via the 6-month assessment tool go to Clinical > 6-month assessment > Enter and transfer records and select "Search for patient ready for 6-month assessment" and tick "Show all teams". You can select the team performing the 6-month review and search for the patient to transfer.
