The post-acute organisational audit will be carried out by SSNAP to obtain further information about the structure and provision of care available to stroke patients once they leave the acute care setting.
This will involve auditing post-acute providers directly about the care they provide for stroke patients. Post-acute providers who offer a service outside of the acute setting to at least 20 stroke patients a year, will be asked to take part in this audit and provide information on the structure and organisation of their service. This will allow SSNAP to obtain insight into the provision of care available for stroke patients once they leave their acute care setting.
In order to do this post-acute stroke services are being asked to register their service and identify audit leads who will be responsible for collecting and submitting the data required.
These are the current timescales for the audit:
- Registration for the audit commences January 2021- 1 March 2021
Data collection commences April 2021 The audit will go live on the 1st April 2021- 30th April 2021 providers are asked to report on their commissioned service.
Checking week: 3 - 7 May 2021 After the data locking deadline, providers will be asked to export their locked data and ensure it is correct.
