The SSNAP clinical audit prospectively collects a minimum dataset for every stroke patient from
December 2012 onwards. This core dataset covers acute care, rehabilitation, therapy and
six month follow up assessment.
Self-reported details of care are provided by different teams across the pathway e.g. acute teams, inpatient rehab, Early Supported Discharge (ESD) teams and community rehabilitation teams.
SSNAP is the single source of stroke data and is the chosen method for collection for stroke measures in the NHS Outcomes Framework and the CCG Outcomes Indicator Set. SSNAP metrics will be aligned with those in the Cardiovascular Disease Outcomes Strategy.
